Advice for extreme shoulder and neck muscle pain?

Ice will restrict blood flow, not increase it. With your situation, you might consider alternating ice/heat (called a contrast bath) which will both address the tightness and inflammation as well as relax the tense fascia. Apply each application for 15-20 minutes or so and then switch, 2 or 3 repetitions at least twice daily. You might also try using over-the-counter Aleve (Naproxen) or Curamin (Turmeric) to help with pain mitigation. These are anti-inflammatories. Give up the massage chair...I've never found one that effectively gets to the area(s) where I need it. Seek out yoga classes from a community college or continuing education group because it will help teach you good stretching, breathing and relaxation techniques. Massages at the school are a good suggestion, but you take your chances with whom you get. Some are really good, and some aren't. With what I consider a last resort, there are pain management techniques such as injections that treat the pain but not necessarily the cause. Of course, everything mentioned is treating the symptoms, but hopefully something will work until you can eliminate the cause.
